How clients can use tech to take charge
When volunteers visit clients in their homes they use a iPad-based survey tool to record the client’s current health and lifestyle  status and encourage the client to set goals for their health. They also help the client set up a Personal Health Record (PHR) which allows the client to add to, monitor and share their health information with members of the care team they select. Together the survey and PHR help the client take charge of their care.
The Tapestry health care team is made up of a variety of health care professionals including pharmacists, nurses, doctors and social workers. They review the report generated by the volunteers after a client visit and determine how best to meet the client’s lifestyle and health goals. They also work to connect the client with the community resources he or she needs to achieve the goals they’ve set.

The Health Tapestry process helps health care teams work with clients to make changes before a crisis occurs. It's an early warning system for nutritional deficiencies, inactivity, over prescription and isolation. The process saves money, is less of a burden on the health care system, and improves the client's quality of life.
Volunteers place summary reports in the client's Personal Health Record (PHR).
The healthcare team links clients with the community activities and resources based on the client’s goals and need.

In this video, Dr. Doug Oliver explains why home visits by volunteers are vital to the Health Tapestry process and how they reveal health and lifestyle details the team might have missed otherwise.
In this video, one of the Health Tapestry volunteers talks about her experiences and what the process means to the clients she visits and assists.
When the care team reviews the report volunteers create after visiting a client the team reviews the goals of the client and tries to match those goals to the community resources in their community that can best help. Those resources may include family members, friends, social workers, therapists and other health professionals. The intent is to make sure clients live full lives and engage with the community as best they can so they can age successfully. 
Health Tapestry is helping clients and primary care teams identify issues that range from mobility to nutrition to mental health.   Whether it’s connecting a client with a skilled occupational therapist for daily functional activities, a dietician to build a new meal plan, or a mental health counselor to help with grief, depression or social isolation….Health Tapestry will help make the best health connections for individuals in need.
Community Groups and Activities
Health Care Professionals
Family members and other in-home caregivers are often  over-worked and under-supported.  Caregiver burnout  is an important, and under-recognized source of stress on  individuals and the health care system. Health Tapestry is working to identify those at risk of  burnout in the home and with the help of their primary care team, connect them to resources and supports  that can help improve their own quality of life.
Hospitals and Clinics
Community service organizations play a key role in the care and quality of life of many older adults all across Canada. Transportation assistance, meal preparation and physical fitness programs are examples of essential services provided by community-based groupss. But, it is a challenge for patients and their families to navigate the often complex web of options that exist in each community. Health Tapestry uses a combination of online and in-person navigation strategies to get patients connected to the services that are right for them.
When specialist appointments and hospitalizations are needed it can be a stressful time for patients. Health Tapestry identifies issues early and has the potential to improve communication between primary care teams in the community and specialists from hospitals. Health Tapestry will help make clients feel supported and will assist with the coordination of important transitions in care by brining the entire care team together.
Family and Caregivers
